Maintain all operations as per NABH standards
Perform sterile compounding of anti-cancer drugs
Provide clinical medication education, especially in oncology, to nurses
Prepare Pharmacy & Therapeutics Committee meeting agenda and circulate minutes
Prepare antibiotic utilization data and present in infection control meetings
Conduct regular crash cart, medication cart, and narcotic audits
Review patient records and enter drug orders using eMAR
Perform drug utilization review and note any discrepancies
Prepare reports on medication errors and ADRs and present in meetings
Submit monthly NABH quality indicator reports
Perform drug utilization review, antibiotic stewardship, and pharmacokinetic monitoring
Conduct regular rounds for patients receiving chemotherapy (day-care & inpatient)
Ensure chemo protocols are followed and address any deviations
Monitor patients undergoing chemotherapy
Detect, report, and assess adverse drug reactions (ADRs)
Provide drug and poison information
Qualifications
Pharm D with KSPC Registration
Must have skills
- Basic exposure to clinical trials / internships in research settings
- Knowledge of SAS, R, or basic statistical tools
- Understanding of pharmacovigilance databases
- Familiarity with medical coding
Good to have skills
- Basic knowledge of biostatistics
- Awareness of regulatory bodies
- Certification in ICH-GCP or Clinical Research
