Job Description

Communicating with patients and their families to provide information about their care plan, treatment options, and other related topics
Coordinating appointments and procedures with other members of the care team, such as physicians, nurses, and therapists.
Maintaining detailed records of patient interactions and care plans, including medication schedules, test results, and other important information
Ensuring that patients are informed and involved in their care decisions, and that their preferences and values are respected
Collaborating with other members of the care team to develop and implement care plans that meet the unique needs of each patient
Monitoring patient progress and adjusting care plans as needed
