Job Title: Home Care Manager (HCM)
Location: Thane
Responsibilities:
- Visit customers at their homes once a month as per schedule or on request.
- Conduct vital checks and update the same in the EMR or tool provided by the company.
- Medication management: Ensure customers have enough medicines as per the prescription, order them if necessary, pouch and organize the medicines for easy consumption, and set up reminders in the application.
- Monitor the general wellness of the customer and update observations.
- Check and update upcoming health checks or doctor visits.
- Conduct a wellbeing call via voice/video once a month.
- Accompany the customer for out-of-home health needs such as doctor visits, hospital visits, or lab/diagnostic center visits.
- Organize resources such as home care services, devices, and medicines based on the doctor's prescription or customer's request.
- Be present with the patient during a medical emergency.
- Support the customer with logistics and insurance claims in case of hospitalization.
- Responsible for customer retention and achieving upsell targets.
- Ensure an impeccable customer experience at all times.
Required Skills:
- Fluency in Telugu, Tamil, and Malayalam (read, write, and speak) is mandatory.
- Presentable with excellent communication skills.
- Ability to engage with High Net-Worth Individuals (HNIs) and their families in a professional and empathetic manner.
- Strong understanding of regional cultural sensitivities and preferences.
- Experience in managing healthcare needs, especially with customers from diverse linguistic backgrounds.
- Must be a paramedicNurse, Dietitian, Physiotherapist, or other relevant healthcare professional.
Qualifications:
- BAMS / BHMS / BDS / BPT / MSW or other relevant healthcare qualifications.
- 3-5 years of experience in a similar healthcare role.
