About The Role
South Downs Leisure is seeking a motivated, empathetic and proactive Exercise Referral Admin Assistant to support the delivery of our Exercise Referral Scheme across West Sussex.
You will play a key role in engaging, processing and supporting clients referred by healthcare professionals to access the scheme in a manor appropriate to the individuals needs.
This role involves collaboration with the Exercise Referral Consultant/specialist and trainers, membership, reception and finance teams, GP surgeries, local councils, and healthcare partners.
Key Responsibilities
- Work with the Exercise Referral Specialists to ensure a smooth & speedy referral process for clients.
- Setting up of inductions on Legend & Referral, diary management of 1-2-1's on Referral & Outlook.
- Process & input referral data on the Referral database.
- Ensure monitoring data is collected & processed to support reporting requirements of ExRef Team
- Contacting clients via e-mail & telephone to maximise uptake of the scheme.
- Liaise with GP surgeries, local health providers and councils to ensure smooth referral pathways.
- Monitor attendance, record outcomes, and maintain accurate data in compliance with GDPR.
- Contribute to monthly and quarterly reviews of the Exercise Referral Scheme.
- Support community outreach and wellbeing events to promote physical activity and health improvement.
