Clinical Pharmacist for Sweden / Austia

We are seeking a Clinical Pharmacist to join our team in India, who will provide medication management and clinical pharmacy services to support patients and healthcare providers in Sweden and Austria. The ideal candidate will have 2-10 years of experience in a clinical pharmacy setting, demonstrating a strong commitment to patient care and safety.

Responsibilities

• Provide expert medication management and consultation services to healthcare providers and patients.
• Review and interpret medication orders to ensure accuracy and appropriateness.
• Monitor patient medication therapy for efficacy, safety, and adherence.
• Educate patients and healthcare staff on medication therapies and safety.
• Collaborate with multidisciplinary teams to develop and implement patient care plans.
• Participate in clinical rounds and contribute to patient care discussions.
• Maintain accurate and up-to-date records of medication therapies.
