Job Description

Coordinating patient care throughout the transplant process, including evaluation, admission, and post-operative care
Educating patients and their families about the transplant process, including potential risks and benefits
Serving as a liaison between the transplant team, patients, and their families
Collaborating with healthcare professionals to ensure comprehensive care for transplant patients
Maintaining detailed records of patient interactions and progress
Participating in quality improvement initiatives to enhance patient outcomes
Adhering to all regulatory requirements and ethical standards in healthcare
Bachelor's degree in Nursing or related field required
Valid state nursing license and CPR certification
Strong communication and interpersonal skills
Ability to work both independently and as part of a multidisciplinary team
Exceptional organizational skills and attention to detail
Previous experience in transplant coordination preferred, but not required
Knowledge of transplant process and healthcare regulations
