Role & Responsibilities
- Conduct psychosocial and financial assessments for patients/clients and their caregivers
- Provide direct counselling, emotional support, and psychoeducation to patients/clients and their families to facilitate coping with illness, grief, loss, and life transitions.
- Collaborate with the multi-disciplinary team (MDT) to develop individualized care plans and facilitate family conferences to support communication and shared decision-making.
- Act as an advocate and liaison for patients/clients and their families, liaising with internal and external stakeholders, community partners, and government agencies to ensure access to necessary resources and services.
- Proactively risks to patient/client well-being and intervene promptly and appropriately to mitigate potential harm.
- Maintain accurate, comprehensive, and timely documentation of assessments, interventions, care plans, and progress notes in accordance with professional standards and organizational policies.
- Actively participate in and contribute to departmental and organizational quality improvement initiatives, service development, and program evaluation to enhance patient/client outcomes and service delivery.
- Uphold the highest standards of professional social work ethics, confidentiality, and practice as guided by the Singapore Association of Social Workers (SASW) Code of Ethics.
Qualifications, Competencies & Experience
- Master/Bachelor/Post Graduate Diploma in Social Work
- Registered Social Worker with the Singapore Association of Social Workers (SASW)
- Good team player with positive attitude, caring and compassionate individual
- Good interpersonal skills, able to communicate with people at all levels such as patient's families, government agencies and other professionals
- At least 8 years of relevant working experience in Social Work sector, preferably healthcare and eldercare setting
- Basic computer knowledge,familiar with MS Office tools and Internet
- Advance Care Planning facilitator would be an advantage
- Ability to speak and write effectively in English.
