Agewell is building a new model of healthcare for older adults—one that combines preventive care, clinic-based consultations, home healthcare, rehabilitation, care management, and ongoing support for seniors and their families.
As we launch our dedicated Senior Care Clinic, we are looking for a dynamic and compassionate physician who is excited to work at the forefront of senior healthcare and be part of a fast-growing, mission-driven organization.
This is not a conventional clinic role. It is an opportunity to build meaningful relationships with seniors, work closely with families, and contribute to a holistic care model that helps older adults age with dignity, independence, and better quality of life.
Key Responsibilities
Clinical Consultations
- Conduct consultations and health assessments for senior citizens at the Agewell Senior Care Clinic.
- Diagnose and manage acute and chronic health conditions commonly seen in older adults.
- Review medications and support preventive healthcare interventions.
- Identify physical, functional, cognitive, nutritional, and psychosocial concerns impacting overall well-being.
Home-Based Medical Care
- Conduct home visits for seniors requiring assessment, follow-up, or ongoing medical supervision.
- Support continuity of care following hospitalization or major health events.
- Provide guidance to families and caregivers on managing health conditions and care needs at home.
Annual Care Plan (ACP) Implementation
- Participate in the development of personalized Annual Care Plans for seniors.
- Oversee the implementation and periodic review of care plans.
- Monitor progress, identify emerging health concerns, and recommend timely interventions.
- Work closely with care managers to ensure coordinated and proactive care delivery.
Multidisciplinary Care Coordination
- Collaborate with physiotherapists, psychologists, nurses, nutritionists, rehabilitation professionals, and care attendants.
- Participate in case discussions and care reviews.
- Help ensure seamless coordination between clinic, home care, and rehabilitation services.
Family & Caregiver Support
- Educate seniors and their families on healthy ageing, chronic disease management, dementia care, fall prevention, and overall wellness.
- Build trusted relationships with clients and families and support informed healthcare decision-making.
Qualifications
- MBBS from a recognized institution.
- Minimum 3 years of clinical experience.
- Experience in General Practice, Family Medicine, Internal Medicine, Primary Care, Home Healthcare, Community Health, or Elder Care will be preferred.
- Valid registration with the Medical Council.
- Ability and willingness to travel for home visits within the city.
