Job Description

Overview

The Pharmacist is responsible for dispensing medications accurately and safely in the OPD/IPD and managing drugs in accordance with hospital policies and regulatory requirements. This role ensures the proper storage, handling, documentation, and distribution of medicines, supports clinical teams, and promotes patient safety and compliance with NABH and JCI standards.

Key Responsibilities

  • Accurately dispense prescribed medications to outpatients and inpatients.
  • Verify prescriptions for appropriateness, dosage, and interactions.
  • Counsel patients on correct medication usage and possible side effects.
  • Maintain records of dispensed drugs in compliance with hospital policy.
  • Monitor stock levels for OPD/IPD
  • Report shortages or expiry of medicines to the senior pharmacist.
  • Assist in stock audits and ensure proper storage conditions (temperature, security).
  • Ensure all pharmacy activities comply with NABH, JCI, and local regulatory standards.
  • Maintain accurate documentation for dispensing, stock movement, and narcotics management.
  • Support internal and external audits.
  • Provide drug information and counselling to patients and clinical staff.
  • Liaise with doctors, nurses, and other healthcare professionals to resolve medication-related issues.
  • Participate in clinical meetings and pharmacy committees as required.
  • Report and document medication errors, adverse drug reactions, or near misses.
  • Participate in continuous quality improvement activities related to pharmacy services.
  • Follow safety protocols.

Experience & Qualifications

  • Bachelor's Degree in Pharmacy (B.Pharm) – Mandatory
  • M.Pharm (Pharmacy Practice or Clinical Pharmacy) – Preferred in tertiary care hospitals.
  • Must hold a valid license/registration from the relevant pharmacy council or board (e.g., PCI in India, MOH license in many regions).
  • 3–5 years of experience in outpatient pharmacy or hospital pharmacy settings.
  • Specific Experience:
    • Experience in dispensing prescription medications, OTC counselling, and patient education.
    • Familiarity with electronic prescription systems (eRx) and hospital information systems (HIS).
    • Handling insurance claims, billing, and drug substitutions based on formulary.
    • Working knowledge of drug interactions, contraindications, and therapeutic equivalents.
