Job Description

Key Responsibilities

Patient Care and Navigation

  • Ensure patients and caregivers feel welcomed and supported during onboarding
  • Coordinate meetings between patients and nutritionists
  • Guide new patients and families through the onboarding process
  • Follow up with patients who miss nutrition appointments and re-engage them
  • Assist in organising support group meetings, activities, and events for patients and families
  • Collaborate with hospital staff, nutritionists, and internal teams to ensure patient needs are met
  • Oversee smooth operations of the nutrition centre and ensure all support services are running effectively

Aid and Support Management

  • Identify and connect with local vendors for nutritional aid and supplies
  • Manage aid distribution, ensuring quality and timely availability
  • Coordinate with the procurement team, manage invoices, and maintain stock levels
  • Identify and onboard patient or caregiver advocates for program engagement
  • Facilitate patient visits, photo/video shoots, and ensure comfort during documentation

Requirements

  • Graduate in any discipline; Master's in Social Work is a plus
  • 12 years of experience in the social sector or community-facing roles
  • Fluent in the local language and familiar with community and cultural context
  • Basic Excel skills and comfort using computers, email, phone, and WhatsApp
  • Must own a smartphone
  • Passion for social change and patient well-being
