Job Description

Roles & Responsibilities:

  • Coordinate Monthly Doctor Consultation Schedule:
  • Collaborate with Program Coordinator to develop monthly calendar for street consultations.
  • Assign consultation slots to targeted zones based on chronic case load and referrals.
  • Confirm internal and external availability 7 days in advance.
  • Ensure transport and logistics are arranged for street visits.
  • Communicate schedule to field teams and update shared calendars.
  • Provide Medical Consultations and Review Treatment Plans:
  • Conduct field consultations weekly or as scheduled.
  • Review vital signs, symptoms, history, and diagnostics.
  • Document diagnosis, treatment, and follow-up plans clearly in field records.
  • Provide prescriptions in local language with pictorial guidance if needed.
  • Review and approve treatment plans suggested by CHNs, ensuring alignment with standard protocols.
  • Conduct Follow-up Visits for Chronic Cases and Provide Emergency Support:
  • Prioritize high-risk chronic cases (e.g., diabetes, hypertension, epilepsy).
  • Conduct in-person follow-ups for complex, unstable, or relapsing clients.
  • Offer phone/video support for CHNs managing stable cases.
  • Train and Guide CHNs and Nalam Workers on Medical Aspects:
  • Conduct monthly training sessions on clinical issues (wound care, chronic disease signs, medication counseling).
  • Review challenging field cases in team meetings for shared learning.
  • Validate documentation in physical records and MIS.
  • Provide practical demonstrations during joint field visits.
  • Maintain training log and track individual competency growth.
  • Offer supportive corrective feedback.
  • Ensure Rational Treatment and Follow-Up:
  • Follow WHO and national guidelines for prescriptions and clinical care.
  • Avoid polypharmacy and unnecessary antibiotic use.
  • Ensure lab referrals are justified and tracked to completion.
  • Review and refine follow-up protocols for chronic clients (e.g., BP/Glucose monitoring).
  • Conduct quarterly audits of prescriptions and follow-up logs.
  • Maintain medical logs and perform periodic stock checks.
