About the role:
As a Medical Chart Preparation Specialist, you'll review and process external medical records to ensure accurate documentation and identify gaps in care. You'll support clinicians with pre-visit prep, quality measure tracking, and chart documentation.
Responsibilities include:
- Reviewing medical records via EMR or outside systems and prepping charts before visits.
- Extracting relevant clinical information and inputting it into the athenahealth system.
- Highlighting trends like frequent ER visits or hospitalizations for clinicians.
- Organizing key documents and lab results for easy access and visibility.
- Managing outbound medical record requests and following up until completion.
- Identifying and recording HEDIS measure gaps using internal tools.
- Completing templates in the EMR to support quality and preventive care efforts.
- Performing other related tasks as needed.

Desired skills and experience:
Required:
- Associate degree, Medical Record Certification (e.g. AHIMA), or equivalent.
- 3+ years experience in health information, medical records, or similar.
- Comfortable using various EMRs and technology platforms.
- Strong attention to detail.
Preferred:
- Experience with pre-visit planning.
- Familiarity with athenahealth.
- Proficiency in GSuite and Google apps.
