Job Description

The NHSGGC AHP Long Term Conditions Service is looking for a motivated, skilled and effective team leader for this permanent AHP Team Lead post to manage the NHSGGC Long Term Conditions AHP Service. This service has developed from the health board wide long covid service. This post has been funded by the Scottish Government Long Term Conditions fund. The team includes advanced practice occupational therapists and physiotherapists, band 6 occupational therapy staff, Health Care Support Workers and administration staff.

This service will build on the success of the covid model. Providing a non-medical/pharmaceutical approach to symptom management, where indicated providing rehabilitation and delivering brief interventions. Where possible taking a digital fist approach using supported self-management techniques. Working collaboratively with service users to utilise brief interventions that promote health behaviour change. This team will work closely with a range of partners including Centre for Integrated Care, Pain Management Service and other services supporting long term conditions including charities and third sector providers.

This is an evolving service, we are therefore seeking candidates with a proven track record in innovative service delivery/development who are skilled to at least SIFs level in Quality Improvement methodologies. You will be a HCPC registered AHP, with leadership experience. Skilled in carrying out service evaluation, with an understanding of service pathways. You will lead your team, providing day to day operational management, such as managing sickness absence and service delivery in line with HCSSA, completion of SSTS and EMIS, as well as other systems as required. You will be responsible for development of your team and provide both professional and line management supervision. The successful candidate will be part of a large AHP community that is firmly focused on staff development, with access to a range of learning and development opportunities. There will be an opportunity to carry a small caseload.

As this is a board wide service access to a vehicle and a full clean driving licence are both essential
