Brief Summary

Join a dynamic healthcare team as a Case Management Officer, where you will provide comprehensive support for heart failure patients and their families. This role focuses on enhancing the quality of care and promoting effective health outcomes.

Responsibilities

  • Conduct thorough assessments of patients medical, social, and financial needs to formulate tailored care plans. Collaborate with healthcare professionals to develop and revise patient care protocols. Lead educational sessions for both patients and healthcare staff focused on heart failure management. Engage proactively with patients through telemanagement, tracking health progress and addressing inquiries. Oversee patient assessments and monitor clinic attendance, ensuring optimal care pathways for chronic heart failure patients. Identify and facilitate advanced care planning sessions for heart failure patients. Participate in quality improvement projects and contribute to the development of clinical guidelines. Provide mentorship and support to junior staff, fostering a collaborative working environment.

Requirements

  • Bachelor's Degree in Nursing or Science. A relevant post-basic qualification in a related specialty is advantageous. Registered Nurse with a minimum of 5 years of clinical experience, including at least 2 years in case management. Demonstrated critical thinking skills and self-motivation. Strong interpersonal, communication, and supervisory skills.
