Job Description

Care Coordination – Implementation of You Are Not Alone (YANA) Programme

  • Develop individualized care plans for all patient types: on treatment, survivors, relapsed, and terminal stage
  • Coordinate with State/Regional Care Coordinators and social/medical support teams for implementation
  • Provide counseling to patients and families regarding treatment plans after consultation with doctors
  • Coordinate inter-unit referrals and track patients until integrated into new care centers
  • Manage incoming referred cases and ensure care plans are in place
  • Connect families with appropriate support systems and external resources
  • Run CanKids helpline numbers in assigned locations and respond to support requests
  • Plan and manage State-level P2L Clinics and SSOPD at Samanwaya Kendras; generate periodic reports
  • Facilitate financial, accommodation, and social support by collaborating with social support teams
  • Assist families with documentation for government and non-government support

Patient Advocacy

  • Ensure protection of patient privacy and dignity across all interactions and programs
  • Collect family feedback and support the measurement of patient satisfaction indicators
  • Contribute to planning and execution of patient-centered advocacy campaigns at state/regional levels

Training and Capacity Building

  • Organize workshops and trainings for Care Coordinators and frontline workers (e.g., ASHAs, Anganwadi)
  • Prepare training content, arrange faculty, manage logistics, and document feedback
  • Act as Master Trainer for Trainee Patient Navigators and oversee their monitoring and evaluation
  • Travel across regions/states to ensure quality training delivery and provide trainer feedback
  • Support content development for training manuals and workshops throughout the year

Management

  • Supervise a team of Patient Navigators in the region
  • Conduct induction trainings for new Patient Navigators
  • Identify and escalate issues or violations to management as required

Administrative Responsibilities

  • Maintain accurate and updated patient records in vCAN (Virtual CanKids Assistance Network)
  • Ensure timely documentation of all case-related information and escalate complex cases
  • Submit weekly, monthly, and annual reports in prescribed formats to relevant departments

Miscellaneous Duties

  • Participate in internal and external trainings to strengthen role capabilities
  • Support CanKids research and impact studies by collecting relevant data
  • Carry out any additional responsibilities as assigned by the management
